Every Person Has Different Needs
Supported Living Services are designed to help adults with developmental disabilities establish and maintain a safe, stable, and independent life in a home they own or rent. California’s Supported Living Services model emphasizes individual choice, personal goals, community participation, and support that can adapt as a person’s needs change.
Because every individual is different, there is no single support plan that works for everyone.
An assessment helps us understand the person’s current abilities, routines, preferences, challenges, and goals. It also gives the individual an opportunity to tell us what is important to them and where they would like assistance.
1. It Helps Us Get to Know the Individual
An assessment is more than a checklist.
It is an opportunity to learn how someone lives day to day. We can discuss areas such as personal care, meal preparation, household responsibilities, transportation, appointments, communication, community activities, safety, money management, and independent living skills.
California law recognizes this importance by requiring SLS providers to conduct comprehensive assessments for the purpose of getting to know the individual and developing an appropriate support plan.
2. It Creates More Personalized Support
The information gathered during an assessment helps identify where assistance may be beneficial and where the individual can remain independent.
Instead of doing everything for someone, Supported Living Services should encourage the person to participate, make choices, develop skills, and work toward their personal goals.
This person-centered approach is also reflected in California’s Individual Program Plan (IPP), which identifies what is important to the individual, their goals, and the services and supports that can help achieve them.
3. It Helps Identify Safety and Daily Living Needs
Living independently involves many responsibilities.
An assessment can help identify areas where additional support may be needed, including preparing meals, maintaining the home, attending medical appointments, managing daily schedules, accessing transportation, preparing for emergencies, or participating safely in the community.
Identifying these needs early allows the support team to plan appropriately while respecting the individual’s independence.
4. It Helps Everyone Work Toward the Same Goals
Supported living works best when the individual, family or authorized representatives when appropriate, service coordinator, and support team understand the person’s goals.
An assessment provides a clearer starting point for that collaboration.
It can also help distinguish between what the individual needs assistance with today and what they would like to learn to do more independently in the future.
5. Support Can Grow and Change With the Individual
An assessment is not about defining what someone cannot do.
It helps identify strengths, choices, available resources, and opportunities for greater independence.
Needs and goals can also change over time. California’s SLS framework is intended to provide flexibility as those needs evolve.
